VGI PARTICIPANT WAIVER, ASSUMPTION OF RISK, RELEASE OF LIABILITY, MEDICAL DECLARATION& MEDIA RELEASE
Assumption of Risk
I acknowledge that participation in hockey training, goaltending instruction, skating, fitness training, dryland exercises, camps, clinics, games, and all related activities conducted by VGI involves inherent risks.
These risks include, but are not limited to:
- Falls and collisions
- Contact with pucks, sticks, boards, nets, equipment, or other participants
- Sprains, strains, fractures, concussions, and other injuries
- Serious bodily injury, permanent disability, or death
Release of Liability
In consideration of being permitted to participate in any activity offered by VGI, I hereby release, waive, discharge, and forever hold harmless:
- VGI;
- All VGI owners, directors, instructors, coaches, trainers, employees, contractors, volunteers, and representatives;
- The hockey complex or facility where VGI activities are conducted;
- The owners, operators, managers, employees, and agents of such facilities; and
- Any affiliated organizations, sponsors, partners, or event organizers;
from any and all claims, demands, actions, causes of action, liabilities, damages, losses, costs, expenses, or legal proceedings arising from or relating to any injury, illness, death, property damage, or loss sustained by me or my child/ward while participating in any VGI activity, however caused, to the fullest extent permitted by applicable law.
This release applies to all present and future claims arising from participation in VGI programs.
Medical Declaration
I certify that I (or my child/ward):
- Am in good physical condition to participate in hockey and related training activities.
- Have disclosed any known medical conditions, allergies, injuries, or physical limitations that may affect safe participation.
- Have no undisclosed medical conditions that would make participation unsafe.
Medical Treatment Authorization
In the event of an accident or medical emergency, I authorize VGI personnel to obtain emergency medical treatment for myself or my child/ward if I cannot be contacted immediately.
I understand that every reasonable effort will be made to contact me before treatment is administered whenever possible.
I further understand and agree that VGI, its instructors, coaches, employees, volunteers, representatives, the hockey complex, and its employees shall not be responsible or liable for any medical, dental, ambulance, rehabilitation, therapy, prescription, insurance, or other healthcare expenses arising from any injury or illness sustained during participation.
All costs associated with medical treatment are my sole responsibility.
Media Release
I grant permission to VGI, its employees, instructors, coaches, volunteers, photographers, videographers, and authorized representatives to photograph, film, audio record, and/or otherwise capture images or recordings of myself or my child/ward while participating in VGI programs, camps, clinics, events, practices, games, or other activities.
I understand that these photographs and video recordings may be used by VGI for promotional, educational, and marketing purposes, including but not limited to:
- VGI's website
- TikTok
- YouTube
- Other social media platforms
- Printed promotional materials
- Brochures
- Advertisements
- Newsletters
- Sponsor presentations
- Promotional video
I understand that:
- No compensation or royalties will be paid for the use of these photographs or videos.
- VGI may edit, crop, or modify images or recordings for promotional purposes.
- Once images or videos are published online or shared through social media, VGI cannot control or guarantee their removal or further distribution by third parties.
